Sprout & Shine DFW – Client Agreement & Liability Waiver
By registering for and/or participating in any class, event, camp, or activity offered by Sprout & Shine DFW, you acknowledge and agree to the following terms:
Understanding of Activities
Sprout & Shine DFW offers children’s yoga, movement, mindfulness, and creative experiences designed to support physical, emotional, and developmental well-being. Activities may include yoga poses, partner work, breathing exercises, games, music, storytelling, art, and relaxation.
Assumption of Risk
Participation in these activities involves physical movement and, as with any physical activity, carries a risk of injury (minor or serious). By enrolling, you voluntarily assume full responsibility for any risks, injuries, or damages, known or unknown, that may occur as a result of participation.
Health & Medical Responsibility
You affirm that you (and/or your child) are in good physical and mental health and able to participate safely. You agree to inform Sprout & Shine DFW of any medical conditions, injuries, special needs, allergies, or medications that may affect participation.
You understand that:
These activities are not a substitute for medical advice, diagnosis, or treatment
It is your responsibility to consult a healthcare provider as needed
You are responsible for determining whether participation is appropriate on any given day
Release of Liability
To the fullest extent permitted by law, you hereby release, waive, and hold harmless Sprout & Shine DFW, its owner, instructors, employees, and contractors from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury (including illness) that may be sustained in connection with participation in any Sprout & Shine DFW activity, whether on-site or off-site.
Parent/Guardian Responsibility
Parents and guardians are responsible for their child(ren) before and after class. For family classes or events where caregivers are present, the parent/guardian remains responsible for supervising their child during participation.
Photo & Media Release
Sprout & Shine DFW may photograph or record classes and events for use in marketing, social media, and promotional materials.
By participating, you acknowledge that you or your child may be incidentally included in group photos or videos. No names or identifying personal information will be used without additional consent, and no compensation will be provided for the use of such images.
Acknowledgment
By registering for a Sprout & Shine DFW class or program, you acknowledge that you have read, understood, and agree to this Client Agreement & Liability Waiver.
